The proposed opening for the unsold pilot for a Dick Cavett show, which (for fairly obvious reasons) never got off the ground.

Here's a link to the article I got the clip from over on Mark Evanier's blog. As Evanier himself says, "If you were producing a show and you wanted to make certain it didn't sell, I'd imagine you'd come up with an opening very much like this one."
